Health Advisor provides insurance information and ways to request assistance. Basic lead information may include name, email, ZIP code and coverage interest. The inquiry form is processed through Google Forms in Health Advisor’s business Google account. Its fields are name, email, ZIP code, coverage interest and permission to reply by email.
The website is hosted through Sites. General inquiries are stored in the business Google Forms account, and new-response notifications are sent to kyle@healthcareproducer.com. Access is limited to the business account and any administrators or collaborators authorized by Health Advisor. We retain inquiry information as needed to respond to your request, manage the business relationship and meet applicable recordkeeping requirements.
